I authorize investigation of all statements contained in this Application for Employment and, as may be necessary, to arrive at an employment decision. I understand that this application is not, and is not intended to be, a contract of employment.

This application for Employment shall be considered active for a period of time not to exceed 60 days. Any applicant wishing to be considered for employment beyond this time period should inquire as to whether or not applications are being accepted at that time.

In the event of employment, I understand I must abide by all rules and regulations of the company.

I hereby acknowledge that any employment relationship with this company is of an 'at will' nature, which means that the employee may resign at any time and the employer may discharge the employee at any time with or without cause. In accordance with IC 22-4-8-4 I acknowledge that I have been notified that my employment by Fun Spot Amusement Park and Zoo is for a specific temporary seasonal period as determined by Indiana Department of Employment and Training Services. As such, I am performing employment as a Seasonal Employee for a Seasonal Employer and my employment is limited to the ending date of the Seasonal Period. Thus, my employment as a Seasonal Employee will deny me coverage under the provisions of the Indiana Employment and Training Services Act (IC 22-4-14-11 Unemployment Compensation)

Furthermore, it shall be the applicant's responsibility to obtain a personal copy of the "Fun Spot Amusement Park and Zoo Handbook" and any applicable departmental handbooks. Additionally, all minors are responsible for abiding by all provisions governing the employment of minors as stated in the Indiana Code 20-8.1-4-1 through 20-8.1-4-32.
